This provides an introduction as to how a healthcare organization might develop a QI project that is based on best practice.

Description of the Assignment Problem and Question
Problem Description
Describe the quality improvement problem that you will focus on in this paper.
(What is the problem related to a nursing practice, policy, or procedure?)
(What is the clinical or practice focus, and why is this issue significant?)
Include any data available to support the need for the examination of the issue.
(What is the supporting data used to identify this as a problem?)
You should clearly state why this problem is significant. This section should be in narrative format and should not take more that two to three clear paragraphs to describe.
The EBP/EBM Question Focused on Quality Improvement
State an answerable question in PICO format and identify its parts (P, I, C, O).
The question should relate directly with your description of problem in I.A.
Selection of Three Appropriate Published Single Studies That Relate to the PICO
In this section of the paper, you need to identify the three articles (each article should be a description of a one quantitative or qualitative study) you are using for the paper by providing complete APA citation and identifying whether the study is quantitative or qualitative. Submit your articles with your final paper.
III. Appraisal of Evidence Grid
Using the grid at the end of this description, clearly identify and describe all components for each study. Submit the grid as an attachment to your paper, but refer to it in the appraisal section of your paper. This should only take one sentence; for example, “The review of three articles is provided in Attachment A; Attachments B, C, and D are the single quantitative/qualitative studies reviewed.”
Address the following questions in your written paper. State each question and then add your critique or responses to the questions. If a question does not apply, state this and explain why it does not apply.
What studies did you select, and are they quantitative or qualitative?
How much variation was there and how was the intervention(s) delivered in the three studies you selected?
How much variation was there in the outcome(s) studied across the three selected studies?
What findings are supported by the selected studies?
What findings are supported by at least one study but not the others, but are compelling? Why are they compelling?
What findings are inconsistent and/or contradictory across the studies?
Of the consistent findings, were any of the studies’ interventions significant?
What outcome(s) was this intervention(s) highly effective in bringing about?
What side effects, harm, or burden are associated with this intervention(s)?
State your conclusion. (This section should be very clear and detailed.)
Using the above critique and the ranking levels described in your text, make a determination of whether sufficient evidence exists to support either current practice or a need to change the practice to improve care. This should then directly relate to Section V of the assignment. NOTE: In a real practice situation three studies would not be enough evidence to make a firm decision.
Describe Change to Improve That Might Be Based on the Evidence You Examined
Describe the change you recommend based on the evidence you have in no more than one to two paragraphs. This should be clear and specific. Based on your brief review of the literature and your appraisal, what is the recommended change(s)?
References
Your references at the end of the paper should include the three studies you reviewed and any other references you used in APA format.
Use the headers and subheaders provided in the assignment description for your paper as your outline for the paper. Include a cover sheet. Use the grid to critique articles and attach a copy of the articles (use PDF or Word format only for articles).
There is no specific length for this paper but it has to include all the sections mentioned above.
APA 6th edition format required.
